Asphyxiophilia
Definition
Asphyxiophilia is a sexual preference or paraphilia in which arousal is experienced through (temporary) restriction of breathing or oxygen supply. Within a BDSMcontext, this is sometimes referred to as breath play . This involves the idea or sensation of oxygen restriction, not actual suffocation.

Explanation of asphyxiophilia
Asphyxiophilia focuses on the tension between control, vulnerability, and intensification of physical sensations. Changes in breathing and oxygen levels can subjectively enhance the experience, for example through dizziness, altered perception, or increased focus on the moment.
Within BDSM, this phenomenon is often linked to power dynamics and surrender. For some people, the thrill is mainly mental: the idea of (temporarily) relinquishing control or, conversely, having it. For others, the physical sensation plays a greater role. It is important to emphasize that asphyxiophilia is not a standard part of BDSM and is consciously avoided by many practitioners because of the risks involved.
Safety & points of attention
Asphyxiophilia is generally considered high risk. Oxygen deprivation can have unpredictable and serious consequences, including unconsciousness, permanent brain damage, or death. These risks can also occur without clear warning signs.
Essential points to consider include:
– Discussing boundaries, signals, and stop procedures explicitly and extensively in advance.
– Realizing that a safe word no longer works if consciousness is lost.
– Avoiding pressure on the neck, throat, and carotid arteries.
– Never combining it with alcohol, drugs, or fatigue.
– Realizing that medical knowledge and experience do not eliminate risks.
Within professional and educational BDSM circles, it is often advised not to practice asphyxiophilia, or to approach it exclusively as a fantasy or role play without actual breath restriction. Mental safety and responsibility outweigh intensity in this case.
